Q distinguishes between the enemy of the people and the enemy of humanity. An Invisible enemy of humanity. At very least it's referring to the few who are so far gone they've lost their humanity, and we aren't aware of who they are. At worst- the V miniseries in the 80's was a documentary. That would put 99% of the population in the hospital.

I figured it was more a metaphor for the libtard "hive mind", like how some of us call them Orcs for being stupid, uncivilized, and often ugly.
