Usually the adverse effect is reported after the most recent dose, so that would be the lot number entered. VAERS is used to report acute vaccine injuries that usually occur shortly after the injection. If the person had 2 shots and 3 boosters and no injury until the 3rd booster, then that's the lot number that would be entered.
