They were growing a new brain for Biden using Fetterman's neck as an incubator. It hatched.

Edit:

They used Fetterman's neck area because it had to develop close to the host's brainstem.
Troll tissue is more resilient than a humans so Fetterman was chosen for the increased chance of organ viability.
Fetterman's loss of speech and cognitive ability wasn't from a stroke, it was Biden's new brain slowly asserting executive control over the host's nervous system.
