This is airy-fairy stuff. We "measure" radioactive decay by (e.g.) Geiger-Mueller counters or scintillators. We have no means of measuring the status of a specific nucleus. We have no means of locating a specific nucleus. If there ever was such a means and method, its application would most likely precipitate the decay event. In any case, the quantum-mechanical system would be the nucleus + measurement. We would still know nothing about the bare nucleus.
