I also got a new dentist because the practice I was at for 25 years was sold. The intake form for my medical history asked a variety of questions, one of which was my Primary Care Doctor’s name. I left that blank since my old doctor was pro-Covid shot and would not see me without a mask, I refused to go back and have not established another doctor. Bottom line, just leave blank whatever you don’t want to share. I have not had anyone ever comment or ask me for the info.
