After watching the NSFW video posted below, it looks to me at the very start of the video you see what looks like a black circle on right chest - possibly making that the entry point and moving towards the throat. This would explain the movement of his chest and body. But there is definitely something off with the video and the blood loss looking like it keeps falling and starting over again.
There are at least two different angles of the event.
