Get the name and license number of the person injecting her, copies of informed consent, HIPAA, NDC number of vial, exact dosage, and have the nurse confirm that it was stored properly, get name of doctor on duty, etc. I think there is a form online, I’ll try to find it.

Also make SURE they give her a vaccine screening form. They are supposed to by law to screen for prior illness. Here’s a copy of the questions they are required to ask:
