I agree there were nursing errors. The manufacturer did not make an error. The incorrect reagent was used to reconstitute the vaccine medication. The company would have liability, I would think. Without being able to track the patients who received the incorrect medication, we'll never know.
You’re presuming to know something that is unknown, though: that the manufacturer wasn’t informed.
They very well SHOULD have been. And in normal cases, they would have investigated, done a risk assessment, and likely removed the subject from the trial.
But yeah nurse fuck ups are like 90% of the fuck ups I’ve encountered. They can’t read simple fucking 16pt Arial font English instructions.
