Spiritual basis for vaccination objection.
I hold a sincere spiritual belief that my creator endowed me with the right to embrace or refuse anything that someone else wants to put into my body. My body is my temple and your mandate on this vaccine is a violation of my spiritual beliefs.
I recently wrote a very brief religious exemption request for a large local hospital network, and it got approved.
I used the information about the use of aborted fetal cell lines in the development of the vax. I then quoted the First Amendment, Title VII of the Civil Rights Act, the EEOC guidance about reasonable accommodations, AND I reminded them that they have been reasonably accommodating and protecting their employees until the mandate, so there is no reason they can't carry on doing it.
I did not mention what religion it was, I didn't quote the Bible, and I didn't provide any other information. Keep it short and remind THEM of YOUR rights.
