Assuming you have a relatively clean health history you can always just get new doctors who you lie to about your identity. It's not really meaningful personal health information when they're wrong about the person it applies to. This will mean you can't really use insurance anymore, but if you're just using a walk in clinic to get some antibiotics you can go out of pocket easily enough.
Hubby went to ER die to accident in another state. Different hospital system entirely. They knew every medicine he took immediately.
If you give them your real name or Social Security number they can do that rather easily. If you don't they'd have to suspect you were lying about the name you used at which point they could run your fingerprints or DNA to figure out who you were, assume you let them take blood.
