Kaiser Permanente has mandated vaccines for all employees with a small window where religious/medical exemptions would be considered.
The initial form in the employee intranet only asked for your beliefs and how they relate to your request. Then they altered it, forcing you to check several boxes before you could submit, stating, essentially, that you acknowledge you are being an asshole for attempting to dodge the vaccine. They also demanded specific verses and sources for your religion to back you up.
The text windows were deliberately very skinny so you couldn't easily review/edit what you'd written, and each had a 1000 character limit.
Also you were running on a 15 minutw time limit to fill it out before the form would boot you and reset itself.

And at the top of the form it says "BEING AGAINST VACCINES OR HAVING SAFETY CONCERNS ABOUT THE COVID VACCINES ARE NOT VALID REASONS TO REQUEST EXEMPTION"
