I put a statement at the bottom saying why I would not sign. HR made my boss talk with me and explain the form! Next came the flu mandate. I claimed a religious exemption which said by signing I understand they may not accommodate my religious exemption request if deemed unreasonable. Next it was they want to talk to me 2:1 about a patient complaint. I said I would not do it without an attorney present and since I was retiring in 2 months I am turning in my resignation effective immediately. Pure harassment and they can kiss my a**. I worked as needed, no benefits, was helping my colleagues out around the holidays so they could be with their families. They just screwed my colleagues because I am done jumping through hoops.
