I have seen so many different templates for religious exemption forms and I know some vary state to state or by industry I haven’t been able to find my solution. I was able to find one for Missouri for my friend who is a fire fighter and it worked and he is not jabbed so I am not being lazy lol I am desperate because he’s getting a jab in three or four days. Thank you very much to this great community ! other than praying to God this is my church and sanctuary and where I go for salvation and to learn about God
