The legality of this is a very fine line. You can't charge extra for women who are of childbearing age, for example as that is discrimination in the group and the point of group insurance is to share and spread the risk.

Early on, I had a wait and see approach because I have a history of seizures. (It is controlled- last one was over 15 years ago.) I was filling out an application recently though and the ADA questions do include epilepsy. Even though that is not my actual diagnosis, it never occurred to me that I could sue for discrimination.
