That is a good question. To some extent, I think it might be exposure...not sure though. Example: I don’t personally know anyone addicted to heroin, however I know there is a thriving black market for it.
Maybe it depends on your position and perspective? If you worked for a 2nd world medical system you might be exposed to, and see the demand more readily.
I think there is enough demand in the corrupt medical industry worldwide to support what we might assume is going on.
Not to say there isn’t some crazy stuff happening at the same time. I think it is also possible that a portion of the distribution channels are for the underground experimental industry (china).
