"Biden doesn't know he is alive" because he is so sleepy and so low energy he's like a dead person.

"he is a different guy" because of the reasons he just gave. he's gone through plastic surgery, his mental state has declined. so he is a different Biden, not literally a body double. But he is actually different as a person.
