Ears cannot be cosmetically altered. They're like finger prints... and can't be changed with surgery. It's impossible for them to go from attached to unattached, and vise versa.
Sorry I should clarify... you can't change certain features of an ear shape easy, or some really at all (without removing it entirely... as you pointed out). Ear lobes are basically impossible to modify, from my understanding... unless you count removing tissue as modification. Changing the shape of the ear folds and so forth under normal health conditions, almost always leads to hearing problems. I just find it highly unusual, at best, that he's changed his earlobes multiple times?
You will also notice several other distinct difference his his overall ear geometry, making some changes even more complex and unnecessary than messing with ear lobes. Some of the differences I don't think could ever be done with cosmetic surgery, again, short of removal.
This is just my understanding of it, from what I've looked into it... I'm not an ear surgeon or anything.

Ears can be, and are surgically altered. You pick up the “ears are like fingerprints” but fail to realize everyone’s shape of the ear is different....
Ask Van Gogh if his ear was altered....
