We will give you a new gaiter to wear if you lack a mask, or we will happily take your order electronically and you can will call it or call us from the parking lot and we'll bring it out to you in good weather.
The language on that would irritate me if I asked you to mask up and you instead gave me that. I 'might' point to my own sign which says 'We reserve the right to refuse service to anyone'.
Just respond politely if we ask you to mask, all you need to say is 'I cannot'. If you must carry a card I would recommend you remove all language personalizing it, let it merely state the fact of ADA and be done.
We will not ask you about why you cannot, we understand it is not our concern. But if you attempt to tell me what I can or can not do in my business I am likely to politely refuse you, leave or you are trespassing. If I read it far enough to see the threats I would not hesitate to inform you we could not help you, have a nice day but have it elsewheres.
Seriously, business owners do not take threats easily, make your card impersonal, just state the law and be done.
Another thing business owners do is aggravation fees; we can and do offer sliding scales on pricing, depending on all factors involved. So another thing which 'could' happen is you throw down the card, I fill your order and my scale slides up, yeah you were a pain and yeah I charged you.

I just hope people realize they're enabling authoritarian government medical tyranny by being the "enforcers" of these ridiculous polices that have nothing to do with a "virus". There are plenty of local businesses near me that have the opposite policy, actually stating masks aren't required on their front door. I've discovered an entire network of these places run by freedom loving people, and happily give them 100% of my business from now on. The prices are higher because they're small, but SO WHAT. I'm glad to pay more to avoid coming in contact with stasi enforcers of the state like yourself. Mind naming your business so I never give you any of my money?

We are a very small business and people seek us out specifically because we are much cheaper than the big boys and we also offer much better service and pricing. But we do refuse to offer service to people who cannot be polite and reasonable, we do so daily.
You seem angry and irrational, we have a Lady up front who enjoys turning your sort around on the spot. She had kids at a school which was shot up some years ago, she doesn't suffer fools well at all.
