Extreme facelift (probably at least two facelifts), eye lift, and massive amounts of injected facial fillers... plus skin thinning due to age explains all of this. His chin changes shapes as he speaks. It always has. Trump's chin makes the same movement. Its easy to see in videos. Attached earlobes are a common result when the skin is pulled too tight during a facelift.
