Hi I know this is an old post I saved it and the reason I’m writing is I used to have a PDF file of the blank card and I desperately need it I’m a single mom with a disabled daughter I need to fill one out . The email was erased off my comp? I know it’s not the best way to do it but we have to do what we have to do ..is there any way you have the PDF file I’d really appreciate it
