Just like European monarchy of old. Arranged marriages for political power. Then they ended up inter-related and the gene pool got messed up.

Side note: I read that before the Tsar abdicated in 1917, because Tsarevich had hemophilia, it would have potentially hurt his sisters' chances at marriage because of the fear that they would be gene carriers. It was a valid fear. When they found the bodies and ran the DNA tests, one of the girls was a carrier.
