b) what you think could have been done to avoid this given the state of play?

Not having a go (neither of your downdoots are mine!), genuinely interested as to the approach as we have our own little hell in the UK here.
If this is in fact a US military operation it would have been trivial to quietly take over these pharmaceutical companies and force them to make placebos.
Why would military intelligence trust globalist corporations - who are already guilty of crimes against humanity for over a century - to inject over half of their population? Why would they simply allow the enemy to open pandoras box on their own gullible population? It's obvious that worldwide medical tyranny / population reduction has been planned for decades, yes? So whatever are in the injections have already been planned for a long time. They can sterilize (already have!). They can maim (already have!). They can use them to initiate future plagues. They can patent their bodies. They can track location/biology. They can install kill switches. They can make them dependent on future injections. Do you see how there is no way to rectify this unless Q is incompetent or complicit?
This is beyond politics. The population is going to believe the lie of the "deadly pathogen" and beg to be saved; a leader can't stop it. But you could placate them by providing placebos and nip the whole thing in the bud.

It does appear that there is pushback of sorts, but you are right- it isn't anywhere near as much as you'd expect if they thought lives were at risk. I will speculate on Q (the actual individuals talking to us). I always got the impression from the beginning that whoever was talking with us was not a decision maker or even had detailed Plan knowledge. This was someone in the intelligence "periphery". They are well-meaning, are military, but are far from "deep" intelligence.
I think now Q have realized that they have been compartmentalized into oblivion. That they were are cog in a bigger machine. They thought Trump was "their" man and he did align with their aim. But he also belongs to higher military intelligence group that subsumes Q. And that group is operating on super-national scale and they are not necessarily benevolent. Trump is compartmentalized in the same way so he was useful to both groups.
So, the military are in fact interested in enslavement (what they would call "modern government") but just not the totalitarian style that the Globalist Ruling Class is aiming for. So all of these counter-Globalist operations are actually just cabalistic in-fighting. Who gets to control the slave population and in what manor?
The key to the serf-class IS the injections. Who gets their nano-tech/synthetic organism in the injection? Who's 5G tech gets installed to track and modify their behaviour? Who gets to decide what demographic is killed/sterilized, etc? This is the struggle. And Q was unwittingly instrumental in this struggle because they were responsible for clearing out puppets that will no longer useful to the higher group in this next phase.

Q has seen the writing on the wall. They are either reconnoitering as a separate entity or left the effort entirely. If "Q" ever come back in the same way then you know for certain that the intention is to bait the patriotic-minded.
