Does any of your churches have a good statement written up on this?

I'm looking at being prepared to give one, but I want to make sure that it is the best that it can be.

https://forunitedsolutions.org/religious-exemptions Scroll down for list of states that have religious freedom exemptions and below that are the form(pdf) needed. My understanding is you need to have it notarized.
