It's a violation of the ADA for an employer to ask personal medical questions for things that don't directly impact your ability to do your specific job you were hired for. You do not legally have to answer whether you have received the shot or not. If they fire you for not disclosing you have a civil rights suit against them. According to the ADA this in no different than them firing you for not disclosing that you had an appendectomy, for instance. It's your own private medical history that they don't need for the job you do and they can't fire you for not disclosing yes or no.
