There’s 2 different ways to say “no” on her questionnaire at her work (won’t disclose exactly what but this job is very close to the medical field). One is “for deeply held religious beliefs” and the other is “I’m not getting vaccinated for reasons other than medical or religious beliefs” Sounds like being honest might be admission of guilt to the max, basically outing herself. What would you do?

A persons religion can literally be one of their own making. I don't belong to a church and I have no interest in belonging to a church. I believe I can worship anywhere I please, and I do not believe in any "authority" who puts themselves between me and God so I couldn't give a shit what some corrupt church leaders have to say about the vaccine. And if some business wanted to pull that shit on me I'd kindly tell them they can go fuck themselves. Those vaccines are an abomination to God and I welcome anyone to debate me on it. Not to mention the vaccine passport system sure looks like the beast system being set up and no one who believes in Revelations would be foolish enough to participate in the beast system.
Also "for reasons other than" means philosophical. There were always 3 types of exemptions, medical, religious and philosophical. You are most protected under the religious exemption because of the 1st amendment. You used to be protected under medical but since science now changes according to the whims of political agendas it's no longer a reliable exemption.
