I know this has been asked before, but what are my options? If I do not put anything down I will be outed. So far they are only asking for dates, not batch numbers. So do I lie, or do I leave blank and wait for someone to contact me and go from there?

This is what I wrote for the religious exemption:
In my recent pre-employment interviews with your company, I asked if there were vaccine or testing requirements, because if there were, I would not work for your company. I was told there would not be a requirement by the your company representatives involved in that process, and that you respect my right to choose. For some reason, this exemption wasn't documented and I am now forced again to state my stance unwillingly, even though I never go into the office and actually work from home out of the state of New Mexico.
I don’t agree with giving your company any personal contact information on top of my Emergency Contact information, let alone personal contact information concerning my religion. This request for information in general is an invasion of my privacy and is, in fact, discriminatory. Your introduction in the email communicating this document specifically says “This commitment includes embracing religious diversity and cultivating a community of inclusion and respect. your company prohibits discrimination against employees and applicants for employment based on religious beliefs, practices, and affiliation.” This statement goes against your requirement for personal contact information of someone that can validate my position, and even you asking for an explanation of my beliefs is a direct contradiction to the statements your company has made up until this point, and as time goes by we will learn that this vaccine requirement is medical malpractice that you and all other companies/politicians in coordination with this will be held accountable for.
Your company should be ashamed of themselves for the undue stress and heartache they are causing their employees from this mandate, and the damage it will cause in their personal lives by either submitting to this deadly vaccine or standing up for themselves. With the shortage in skilled healthcare workers that was spoken about in the same sentence as mandating the vaccine with the threat of loss of employment in the last townhall, this will surely cause a loss in employees and a decrease in income for the company. The quality of work will drop drastically, customer satisfaction will decrease, and honestly, it is a patient safety issue that is being ignored.
The death rate is low, there are alternative medicines that treat it, and I prefer to live a healthy life focusing on my own natural immunity instead of taking never ending unapproved and untested vaccines. Healthy living is what I have done my whole life and I never get sick, my family as well. We exercise, eat heathy homecooked meals and take natural vitamins, we get a lot of sun and have a lot of fun. This is the true pathway to a healthy lifestyle but I haven't seen one thing about anything like this coming from your company or any other hospital or public health institution at all, almost like you want the people to get and to stay sick (especially when you think about the mask mandates for the last year and a half and the obvious unwillingness of these groups to speak about the dangers of wearing masks for long periods of time including bacterial pneumonia and psychological issues). If you were truly trying to save lives you would mandate proper diets and exercise instead of masks and vaccines.
In addition, "variants" are showing up and massive boosts in the number "cases" coincidently happening directly after the vaccine rollout, and the refusal of hospitals and labs to speak out about the possibility that the vaccine is the cause, is unacceptable. The unending insanity of how the medical world, the media and the government has handled this virus from the beginning with perfect coordination on masks, lockdowns, and forced vaccinations without any corresponding healthy living advice… the blocking of any dissenting opinions in the news, social media, politics and in your own medical circles… I have lost all faith in the healthcare system, and there is no chance you will ever force me to be injected with another thing. Healthcare institutions and labs across the country should be fighting to gain back the trust of the people and prove that they are capable of handling real crisis and actually follow their own scientific research to make decisions and statements instead of following politics and popular opinion and manipulating data to get there.
A very simple game of logic will help you understand why this vaccine push is not logical and is only being pushed as a way to control people. If the masks worked, why did we have to separate 6 feet from each other in the beginning and still to this day? If the 6 feet rule that hasn't changed since day one worked why wear a mask? If they both work why was everyone locked down for so long for no reason? If all of those worked, why did we have rushed experimental vaccines with faulty trials? If the vaccine works and is safe, why is there a no liability clause for the creators and the mandators of the vaccine? If more people die from the vaccine than the virus, why use the vaccine? Why do the vaccinated have to wear masks? If fully vaccinated people can get and transmit the virus and the numbers increased post vaccine how is it that the non-vaccinated are the super spreaders? Basic logical questions will pull you out of your group think and get you thinking like a regular person again and hopefully you will see why so many people have issues with the whole charade.
As far as the religious exemption, of course, I have a deeply held religious belief against injecting myself or my family with any man made "vaccine", especially ones that use fetal cell tissue. I am permitted this exemption under Title VII of the Civil Rights Act.
