I know this has been asked before, but what are my options? If I do not put anything down I will be outed. So far they are only asking for dates, not batch numbers. So do I lie, or do I leave blank and wait for someone to contact me and go from there?

Just don't reply. Buy yourself some time. You're going to get fired eventually, I'm sorry to say, unless you can swing a religious exemption excuse.
But who cares? Obviously none of us know your situation and odds are you -need- this job to provide for yourself or your family, but I'm a firm believer that our thoughts create our reality and that if you seek God's wisdom, He will help guide you.
So, pray and plant your feet firm in the ground. Make them come to your office or desk and ask you in person. Then tell them you'd prefer not to share that information, because it is private medical information that should only be known by you and your doctor.
Hang in there, keep your phone ready to record for when you get called into the boss' office, and accept whatever may comes.
Remember that you don't even want to work there in the first place. Who wants to work somewhere where they're surrounded by tyranny and communist monsters?
You'll find a new job, especially when patriots dominate the field again.
