Typically its not illegal to ask, BUT it can lead to discrimination claims down the road. If she says "no" and the basis of the no is a protected class (religious reason or medical health reason) and then they take action against her it can be deemed religious discrimination.
My recommendation is to not lie. Either say "no" or "will not disclose". If it's a one-party consent state, they might consider taping the conversation.
Edit: If truthful, you might even recommend saying "no, based on my religious beliefs" to put it squarely in the protected class status.

Check the employment contract. If there's nothing in it about vaxx then she has solid legal ground for nearly anything
