A note on the Polio vaccine: it went through 3 iterations.
The first, developed in the early 1930s maimed/paralyzed/killed ~100 children due to faulty development and testing.
The second (by Salk) in the 1950s killed ~250 children and paralyzed many more because of a production/quality control error.
The third was the safest of the group - but the first two had damaged the public's trust to the point of vaccine hesitancy. After many years of reluctance, mandates were enacted in the 60s (a full 30 years after the first vaccine was put into use.)
We should further note: all three versions were considered "safe" by the Government.
