I recently met with a lawyer who specifically said not to submit a medical AND religious exemption.
ONLY SUBMIT A RELIGIOUS EXEMPTION
Anyone who tells you differently is completely uninformed.
Religion is considered a “protected class” under the civil rights act. Medical stuff is not.
So what you need to do is write a religious exemption in your own words—one that is SINCERE.
Employers can reject your letter if they discover you copy&pasted from a template.

Please head this warning. My mother in law was rejected because she submitted a medical exemption before her religious exemption. She was fired.
I am more of a don't jump through hoops kind of guy. I will not be engaging in any applications for exemptions for any reason, I just do not do vaccines, and leave it at that, come what may. It's not that I can't, it's not that I won't, it's because I don't.
I do not is final, requires no elaboration. Maybe they make a decision, one way or another, but I don't do vaccines.
My office was very anti-discussing the jab until two weeks ago. My boss is based and hates the whole topic, but word came down from upon high and now it's mandated.
My boss sent around an email announcing that we all had two weeks to show evidence that we had taken the first shot. He then said he would be sending a follow up email 72 hours later and if you get that email you still need to show evidence. I did not get that email and he seems to have left some people off of the list.
