Probably he got the vaccine into his vein instead of muscular tissue. This risk is way higher if the nurse hits a vein. It MUST be administered into the muscle. Nurses need to "aereate the needle" meaning pulling back a bit to see if blood is coming out, if so, abort. They are instructed NOT TO DO THAT ANYMORE! I don't see anyone talking about this.
