I would be careful when submitting these. At my job, they've started actively searching for exemption form letters online so they can compare them against each religious exemption they review. If they find that you submitted a word-for-word or minimally-altered form letter, they'll reject you outright because they'll say you aren't being sincere.
It's fine to use one as a foundation/guideline, but you should always have it written in your own words so they can't use one of these shitty loopholes against you.

Except they’re making everyone resubmit their request no matter what b/c my coworker and I didn’t look at a template and used our own words and we have to resubmit our request and provide even more info even though we were originally approved. Yeah, our company is run by a bunch of corrupt lefties. 🙄🤦🏼♀️

We had to use a form that would reset/clear itself every 15 minutes so that you would feel rushed.
The fields were purposely incredibly narrow to make proofreading/editing difficult. They were also limited to 1000 characters after they demanded in-depth explanations from you.
We had to check boxes at the top that stated we acknowledged vaccines were good and we were bad for questioning them. This was mandatory, no option for Under Duress or to disagree. You could not submit the form without checking these boxes.
And then after all that, they had the absolute nads to whine in a managers' email that "BAWW our employees are communicating and coordinating online to help each other write exemptions and what language to use so we can't deny them!"
Bahaha! Yup! You guessed it, and the Union has done crap all to stand up for us. 🙄 It sounds like you might be in CA though b/c while the questionnaire sounds similar, I don’t remember having to check any boxes saying the vax is good. 🤔 I might not have realized though, that that’s what I was checking to continue. Lol. I spent hours writing up my request on a word doc only to realize when I went to transfer my paragraphs that there was zero room to thoroughly answer the questions they asked. So then I had to majorly edit everything which also took forever! 🙄🤦🏼♀️ This last set of questions were downright insulting and bordering on harassment. They were insane! Based on those questions I get the feeling that they aren’t worried in the least about lawsuits or getting sued into oblivion for religious discrimination. They think they’re untouchable. 🤷🏼♀️ Good luck, fren! If you’re in CA, have they responded yet to people’s “resubmission”?

I'm wondering if things are moving behind the scenes, though, in light of the lawsuits being brought against them by AFLD. When my exemption was initially approved, I was told I needed to submit to bi-weekly testing as a condition of it and that my manager would be following up with me about it. After that, I heard nothing. Nada. Even followed up with my manager to make sure she hadn't forgotten.
Now we're being told that testing will no longer be conducted on unjabbed employees.
I haven't received an email (yet) demanding further information about my exemption, but they seem to be going out in batches every Friday. For now, my HR case still reads as "Exemption Approved", so I'm going to hope I'm not worth pursuing since I don't directly work with patients or staff, and it would be difficult to argue that its hard to accommodate me.
