Great theory! She was the director of photograpy and the other was the director (who survived) Why would be pointing at people behind the camera?

Afaik, they haven't used real guns with blanks in a long time, just prop guns with a muzzle blast, or put in post production. I am intrigued about the real story behind this psyop.

Same as Brandon Lee in the Crow. There is nothing accidental about putting live ammo into a prop gun.
REALLY it shouldn't ever happen - blank ammo is NOT the same like real one.
Accidents where such thing happen could be probably counted by fingers of one hand.Very rare.
