For a blank to kill someone, you have to have the muzzle extremely close to them. If she's behind the camera, and the gun is in the shot, that would be too far away. It also would be one and done, and couldn't injure anyone else unless it was intentional and he moved the gun around to people and continued to fire at them.
This was most likely the result of live rounds. By the way, most "prop guns" that shoot blanks, aren't "prop guns." They're guns loaded with blanks.
I believe Bruce Lee's son was killed in a similar way (live rounds on set). But there's no way live rounds accidentally find their way onto a movie set. I doubt Alec planned this, not that it's impossible, but more likely someone used him -- though the intended target would be questionable.

In Brandon’s case a shell casing was lodge in the barrel supposedly. Or the lee curse/assasination. I alec fired twice had to be real ammo.
