I got exactly what you are looking for by going to the FLCCC website and looking up a doctor in my state who is listed as willing to write a prescription for Ivermectin. I figured that criteria might indicate that he/she would not be a brain-dead follower of the CDC guidelines.

I found a doctor in my home state (AZ) and messaged him through the Medici App. I gave him the details of my medical condition and let him know that I had already had Covid and successfully treated myself with Ivermectin (horse-paste version). I stated that I didn't think the vaccine would be either safe for me or even necessary. He agreed and wrote me a letter and filled out the exemption form that my employer was mandating. This was all done by telemedicine, I didn't have to leave the house. The total cost for the consult and paperwork was $97.
