It is none of their business. All questions can be answered with: "deeply personal and religious reasons".
Employers making decisions about one's religious beliefs?? What qualifications could they possibly have to do that?? Attempting to do that is itself a violation of the 1st Amendment and God's creation.
Just because they print up a form does not make it valid. They might still "enforce" and that is when you need an attorney to keep your job. But you do NOT have to give in to their egregious questions.
I know it is not easy out there! Prayers to all facing tyranny, which trades feeding your family for conscience. I noted that was one of the most evil aspects of the totalitarian communist and nazi regimes. FORCE one into evil against conscience merely to live!!
