I use that phrase when I would normally keep my mouth shut. Telling someone a brutal truth. It's my way of saying "don't let this hurt your feelings."

"I'm going to be completely open with you" would get your point across without the negative connotations.

Some people assume those who use the phrase, "I'm going to be honest with you," are habitual liars.
