I think he will not die, but be diminished and it will be the last straw to 25th him. Then he disappears into extended care forever.

Maybe not die right away, but maybe comes out with an unexpected cancer diagnosis and needs to retire immediately and focus on his health.

Something like that. Or he could plausibly have "an unexpected reaction" to anaesthesia and just not able to get his thoughts together as he usually does. Too many people haven't noticed how incompetent he really is. This makes them pay attention.
