I tell them that I will wear one (out of respect) if they have a mental illness.

I was in a store the other day with literally no more than 4 or 5 other customers in the entire place, plus employees. It didn't take long for one of them to birddog me to the back of the store and say, "Ma'am, do you have a mask?" I smiled and said, "I can't wear one, Sweetheart". She paused, mouth agape, and said, "Ok".
