What does “fog of war” mean? Is that a euphemism for fake?

It is a term that refers to the confusion often found on a battlefield. Smoke everywhere on a dense foggy morning; fighters have a hard time seeing friend from foe, or being aware of what is happening nearby. Hence the term "fog of war" used to describe a situation that is difficult to figure out friend from foe; or reality from illusion.
