Not as far out as you think. Sewage treatment plants in the US must treat water to drinking water standards (Tittle 22), but trace elements of pharmaceuticals make it into the effluent, which typically is pumped into a body of water like a river, in which case downstream cities will take up the same water, treat it and send it to customers. The trace amounts of pharmaceuticals can make it into the drinking water.
