That's reportable to the PA board, and actionable. False documentation is a big no bueno for every medical professional. Good luck, i encourage you to follow through with a board complaint. This won't stop until we stop it.
oh yes. False documentation is like ... a murder one charge, in the medical world. You should be able to report it right on the website for PA. AND most states require PA to be physican supervised, meaning that the doctor who signed off on that false documentation is also responsible.

He did ask if I wanted a strep test which I declined and in his notes it said “ I have ordered a strep test and you can find your results on our website in a few hours.” I told my husband “I don’t know what he’s testing for strep, he never took a swab!”
Looks like ‘Broken Glass’ coof is the new pandemic. Just went through the same, but didn’t go to the urgent care. I’m seeing a lot of these cases. It progressed to sinuses then a weird cough at night on 7th day on.
The question is why? "Maybe" (I have no idea), they don't want the cooties data pumped up or maybe if they get a + cooties they are forced into a bad protocol. It is really odd for sure. Just not sure if it's good or bad. Maybe they are just tired of talking about it. :-)
I incline to that idea. Such reports are very stereotyped to be legally "enough" without getting into details that would be trouble later. This PA has no tests to go on, he can only consider the possibilities and the actions that would generally be helpful. He's putting it on the patient for making him guess, at the same time the PA may know with 99% certainty that it's a strep throat and wants to save the patient time wasted on some covid nonsense when he needs an antibiotic right now.
I think there are more and more medical people waking up and trying to avoid the crap all together. I think they see the storm coming and don't want to be on the receiving end of any flying debris. They are quietly backing away from the door frame.

Be a little dramatic with your verbiage. The complaint screeners are usually not professionals.
