They actually do depending on the weapon used and distance.

I've seen maybe 25-30 heads that have been shot with a firearm, only ones that ever "blew up" were by shotgun. I trust my eyes and experience. I would imagine a large caliber "hunting" rifle or .50 cal would also do that, just haven't seen that myself.
