You don't have to i use it on my 90 year old Dads back and it is brilliant. Don't dilute it just a few drops on a cotton wool ball. Also used it on arthritis in my finger that was interfering with my Guitar playing and it did the trick. Alas i now have no excuse for my bad playing ;-(
