Check out Where Men Win Glory by Jon Krakauer. Excellent non-fiction account that gets into the friendly fire incident, and also shreds McChrystal.

Incidentally I watched War Machine the other night. SPOT ON satire of the whole gov-mil industrial complex down to the simple dialogue and hand movements. Incredible, the only reason I can think why it has a 6.0 on iMDB is most people probably think it's ridiculous. Well anyone that's spent time in the military or DoD civilian will get deja vu from some of the scenes in the movie. 10/10 for accuracy and Brad Pitt is hilarious as always.
