But surely an ex-president isn’t on Medicare and doesn’t need free care, as he’s already getting it right?

Medicare isn't free! You pay a monthly fee based on income. You also pay for services not covered by Medicare. However, he probably is entitled to some medical care. What is it? Does that cover hospice?
