My late wife had hospice care and they gave her a morphine drip and when the pain got too much she squeezed the lil' ball...BUT she was NOT lucid at the end due to the pain meds...
Believe it or not, I KNEW she was going to go the day I came home from work and at 5:05 pm my father-in-law came out in a rush from the bedroom and I remember going into the room and raising my head and saying "Thank you Lord for taking her."
I think PAIN meds SHOULD ONLY be given if the patient is lucid and able to communicate...My wife was and we HAD DISCUSSED the possibility of pain meds...I followed her wishes to the letter and never looked back because it was in writing what SHE WANTED NOT some damn doctor pushing the stuff!!!
