There are mandatory reporting diseases already- HIV, STDs, Tb, previously "eradicated" diseases, etc. If a case is confirmed pos, the doctor/clinic/hospital/lab reports that to States' health boards- thats how we find out about "outbreaks"- that data is rolled up to the CDC...search the posters at ur drs office, hospital lobbies- you will see some sort of sign communicating that certain diseases are reported to ur state agencies
