Here's a possibility, and I say this because I did this too back in the day. I wasn't ABOUT to get the jabs, so I printed off my own card on my printer and entered bogus dates and info. Then I realized that "they" weren't going to come for me and that vaccine passports was a stupid idea, especially in my home state of Tennessee. It never happened.
A week ago I stumbled across my old made-up card and time had not been kind to it. Ink was flaking off and it looked SORTA like the image in your picture above. I see where the ink is faded, whereas a real CDC card was printed professionally.
COULD IT BE (ahem!) that your fiance printed up her own card like I did, and this is what you stumbled upon? I'd ask her about it, since your health is involved.
I found out that when you walk in to get your vaxx, the first thing they do is fill out your vaxx card & hand it to you while you wait for someone. Then you can just leave, it's not illegal.
