Here's a possibility, and I say this because I did this too back in the day. I wasn't ABOUT to get the jabs, so I printed off my own card on my printer and entered bogus dates and info. Then I realized that "they" weren't going to come for me and that vaccine passports was a stupid idea, especially in my home state of Tennessee. It never happened.
A week ago I stumbled across my old made-up card and time had not been kind to it. Ink was flaking off and it looked SORTA like the image in your picture above. I see where the ink is faded, whereas a real CDC card was printed professionally.
COULD IT BE (ahem!) that your fiance printed up her own card like I did, and this is what you stumbled upon? I'd ask her about it, since your health is involved.
I was ready and willing to stand and claim I never got a vaccine anywhere I went. Still am and did. Cops called me for not wearing a mask. Almost lost the job for not getting it. Went face to face with a federal doctor and told him no. Went face to face with a fed and said no. Face to face with family too.
Not finding fault with you, because I know others who did similar and we can only do what we can. We all are human and have families. Me too. But the none-sense pissed me off and I have to be honest. Those who did not stand up but hid pissed me off too. Why? It would have been over the next day if anyone was like me and I am not as intelligent and gifted as those as you.
Next go around I hope more have the back bone and courage to flip off the world too instead of trying to fit in and hide. Especially in the USA.
I totally understand though. One guy I know has many kids and is the only source of income and did not take the mRNA but did have an official vaccine document from the hospital.

I wouldn't be too sure about that. My son travels a lot for work, including to countries that were super hard ass about producing a Covid test to fly and proof of taking the shot. He ginned those things up with no trouble at all, even on his phone at the airport.
I found out that when you walk in to get your vaxx, the first thing they do is fill out your vaxx card & hand it to you while you wait for someone. Then you can just leave, it's not illegal.
